What the service does

We provide support for people aged over 18 who have long-term mental illness and are receiving help from a community mental health team.

The care management team supports and advises you on:

  • services that help you live at home
  • services that promote your independence

These services include:

  • domiciliary care
  • self-directed support
  • supported housing
  • residential and nursing care

How to access the service

We accept referrals and assessments from the community mental health team. We also need information from any health professional you are currently seeing or working with.

Monday to Friday from 9am to 5pm (excluding public holidays).
