This important new resource has been co-produced in partnership with people living with dementia, carers, Alzheimer’s Society, Dementia NI, and Trust staff. It is designed to support people living with dementia and those who care for them.
We are asking patients and their families to work with us to ensure they leave hospital within 48 hours of being declared medically fit to do so by a doctor.
