Leaving hospital

Where possible, we will make plans for your discharge from hospital when you are being admitted. In some cases, planning may begin before you are admitted. It is important that your discharge is not delayed after your treatment is complete.

On the day you leave hospital, you may be asked to wait in our discharge lounge for your family, carers or friends to collect you.

What we will do
  • We will discuss your planned date of discharge, your transport options and together agree your arrangements for leaving hospital. We will involve your chosen relative, carer or friend as appropriate.
  • We will discuss and agree any equipment or support you may need.
  • If you need to be transferred to a transitional care bed, we will send you to the first available bed. The priority is to start your rehabilitation as soon as possible.
  • We will discuss your medicines with you. Any medicines you brought into hospital will be returned to you if they are still suitable for you to use. Please feel free to ask about changes to your medicines or why medicines have been stopped.
  • If we have started you on a new medicine, you will be given a limited supply to take home. You need to contact your GP if you need a further supply. Please ensure you do not run out.
What you should do
  • You should arrange your own transport home. Hospital transport is for people with assessed medical needs only.
  • Please remember your outdoor clothes.
  • Please ensure you have access to your house keys.
  • Please ensure your heating has been turned on at home.
  • Please ensure you have enough food at home.
  • Please let us know if you require a medical (sick) certificate before you leave hospital.
  • After you return home, please contact your GP or pharmacist if you have any questions about your medicines.
