What the service does

The community oncology and palliative care service supports other community nursing services. We work with patients and families who have complex needs resulting from cancer and other conditions. Our work helps prevent unnecessary hospital visits.

We improve the quality of life of patients by:

  • quickly responding to referrals
  • offering guidance on how to control symptoms
  • offering advice on how to access services
  • providing expert clinical support and education to other healthcare professionals
  • working in partnership with other organisations involved in the care of patients with cancer and life-limiting illnesses

The community oncology and palliative care team is made up of:

  • doctors
  • nurses
  • social workers
  • physiotherapists
  • occupational therapists
  • speech and language therapists
  • dieticians

How to access the service

We accept referrals from:

  • hospitals
  • GPs
  • other healthcare professionals
