What the service does
The Community Rehabilitation Team supports people who have a mental health condition that seriously affects their ability to lead a meaningful life. We help patients develop the skills to live, learn and work in the community.
We encourage patients to:
- identify and develop their strengths and goals
- build confidence, independence and skills
- manage their mental health through education and interventions
We focus on:
- improving daily living skills
- creating a relaxing living environment
- scheduling regular activities
- making and maintaining social contact
- developing intimate relationships
- pursuing education and career opportunities
We work with people who have been in hospital a number of times and have not recovered with the help of generic mental health teams. Our comprehensive approach can help address their complex needs.
Who provides the service
The Community Rehabilitation Team is made up of:
- consultant psychiatrists
- speciality and trainee doctors
- community psychiatric nurses
- occupational therapists
- social workers
- psychologists
- support workers
How to access the service
We only accept referrals from community or inpatient mental health teams.
