What the service does

The Community Rehabilitation Team supports people who have a mental health condition that seriously affects their ability to lead a meaningful life. We help patients develop the skills to live, learn and work in the community.

We encourage patients to:

  • identify and develop their strengths and goals
  • build confidence, independence and skills
  • manage their mental health through education and interventions

We focus on:

  • improving daily living skills
  • creating a relaxing living environment
  • scheduling regular activities
  • making and maintaining social contact
  • developing intimate relationships
  • pursuing education and career opportunities

We work with people who have been in hospital a number of times and have not recovered with the help of generic mental health teams. Our comprehensive approach can help address their complex needs.
