Bed Based Rehabilitation
Bed based Rehabilitation is a short-term inpatient service providing 24-hour care for people who are medically fit to being discharged from hospital, but cannot be discharged directly home, as they require a further period of rehabilitation and support.
The multidisciplinary team made-up of nursing, medical, physiotherapy, occupational therapy, social work and pharmacy, provide 4 weeks ongoing assessment and work with the person to achieve their rehabilitation goals. Bed based rehabilitation can help to avoid a person going into hospital in the first place, or provide somewhere to recover if a person is not initially able to return home.
Who the service is for?
Adult patients living within the Belfast Trust area who require 24-hour assistance and further rehabilitation following an admission to hospital.
How can you be referred?
Once medically fit referrals are made from the hospital discharge team, medical consultant or hospital social worker, when a person requires a period of further 24 hour support and rehabilitation.

Home Based Rehabilitation
Home-based rehabilitation is a community-based service, which provides rehabilitation and support in a person’s own home.
The team consists of Occupational Therapy, Physiotherapy and & Rehabilitation Support Workers.
This service enables a person to be discharged from hospital, once medically fit, to their own home by providing further support, assessment and rehabilitation.
Individual needs and rehabilitation goals agreed with the person assist them back to improved health, function and quality of life. It is a time-limited service lasting up to 6 weeks.
Who the service is for?
- Adults who reside in Belfast Trust area
- Adults requiring assessment and rehabilitation once medically fit for discharge
How can you be referred?
Once medically fit referrals are made from the hospital discharge team or hospital social worker
