Condition Management Programme

What the service does

The Condition Management Programme (CMP) offers short, work focused interventions aimed at helping people understand and manage their health conditions.

Our team of experienced healthcare professionals works together to support people with single or multiple health conditions that are causing a barrier to work. Depending on your needs, you may work with one or more health professionals.

We provide advice on:

  • dealing with stress, anxiety and low mood / depression
  • coping with pain and fatigue
  • relaxation techniques
  • communicating with confidence
  • developing a healthier lifestyle
  • exploring potential work options

You may be eligible to participate in the Condition Management Programme if you:

  • are struggling to stay in work due to your health condition
  • have recently gone off work due to your health condition
  • are receiving benefits and struggling to find work due to your health condition

If you are currently in work and are receiving support from Workable (NI) or Access to Work, you can also participate in the Condition Management Programme. You can ask your Case Manager to refer you.

Who provides the service

Our team is made up of:

  • occupational therapists
  • physiotherapists
  • specialist mental health nurses

How to access the service

You can ring the office on 028 9615 2688 and refer yourself to the Condition Management Programme.

You can also get a referral from:

  • your work coach at your local Jobs and Benefits office
  • a healthcare professional you currently visit
