What the service does

Diabetes specialist teams are based in the community. They provide the following services to people with diabetes and their carers:

  • specialist care
  • treatment
  • support
  • education

Diabetes specialist teams work with a range of patients, including:

  • vulnerable adults
  • people with learning difficulties
  • people whose diabetes is complex and difficult to manage

Diabetes specialist teams offer:

  • clinics in health centres and wellbeing and treatment centres
  • visits to patients’ homes (when appropriate)
  • holistic management to reduce the risk of diabetes related complications
  • glycaemic management, including oral antidiabetic drugs, insulin initiation and titration
  • specialist dietetic advice and support
  • specialist podiatry clinics
  • personalised education programmes
  • one-to-one diabetes education sessions

Who provides the service

Diabetes specialist teams are made up of:

  • nurses
  • dietitians
  • podiatrists
  • healthcare assistants

How to access the service

We accept referrals from:

  • hospitals
  • GPs
  • other healthcare professionals
