What the service does
Diabetes specialist teams are based in the community. They provide the following services to people with diabetes and their carers:
- specialist care
- treatment
- support
- education
Diabetes specialist teams work with a range of patients, including:
- vulnerable adults
- people with learning difficulties
- people whose diabetes is complex and difficult to manage
Diabetes specialist teams offer:
- clinics in health centres and wellbeing and treatment centres
- visits to patients’ homes (when appropriate)
- holistic management to reduce the risk of diabetes related complications
- glycaemic management, including oral antidiabetic drugs, insulin initiation and titration
- specialist dietetic advice and support
- specialist podiatry clinics
- personalised education programmes
- one-to-one diabetes education sessions
Who provides the service
Diabetes specialist teams are made up of:
- nurses
- dietitians
- podiatrists
- healthcare assistants
How to access the service
We accept referrals from:
- hospitals
- GPs
- other healthcare professionals
Location of service
Diabetes specialist teams are based at:
- Howard Building, Twin Spires, Falls Road
- Knockbreda Centre
Clinics are also held in:
- Arches Wellbeing and Treatment Centre
- Bradbury Wellbeing and Treatment Centre
- Shankill Centre
- Whiterock Health Centre
