What the service does
Hospital social workers support patients, carers and families who are dealing with the consequences of illness and/or disability.
They help patients organise the services they will need when they leave hospital. This ensures patients can be discharged from hospital safely and on time.
If a patient is unable to return home, our hospital social workers will help them find alternative accommodation such as a nursing or residential home.
How to access the service
If you are in hospital as an inpatient and need support, you can refer yourself to the social work department. Ask any member of the medical or nursing team.
Families or carers can refer an inpatient by contacting the social work department in that hospital.
