Raise Awareness and Identify Carers within Families and Communities

We will achieve this by working towards the following indicators:

Exploring How Carers are Identified Across Belfast Trust Service Areas

  • Improving the identification of the carer population in Trust service areas
  • Raising the Belfast Trust Carers Register by 10% each year
  • Reviewing how the implementation of Encompass supports the identification of carers in service areas and electronic referrals to the Belfast Trust Carer Register

 

Enhancing Communication, Promoting Diversity and Inclusion by Ensuring a Wide Range of Groups and Carers are Identified

  • Developing a Carer Communications, Diversity and Inclusion Plan to reach all carers, stake holders and promote support services for carers
  • Delivering a ‘Carer Aware Campaign’ across the Belfast Trust area to reach all stakeholders and the wide diversity of carers of all ages and cultural backgrounds
  • Establishing links and partnerships with all stakeholders to improve communications and reach all carers in Belfast

 

Promoting Recognition and Understanding of the Carer Role

  • Delivering a co-produced ‘Carer Awareness’ staff education programme to:
    • Improve the understanding of who a carer is, their rights and why it is important to identify their support needs, and
    • Enhance the quality of the experience for carers through better day-to-day interactions resulting in better care for the service user
  • Communicating the findings of carer assessment audits with staff to make improvements

 

Enhancing How We Provide and Communicate Carer Information Across Belfast

  • Developing Carer i-Hubs (One Point of Contact for Information) to provide carer information across Belfast Trust service areas
  • Providing information on Belfast Trust Carer Support Service and the Carers Register
  • Engaging with carers to make the right information available in service areas

 


We will measure our progress by:

  • Increasing the carer numbers on the Trust Carer Register by 10% each year
  • Auditing and monitoring implementation of Carer Action Plans in service areas
  • Auditing the number, quality and recording of Carer Assessments offered, completed and declined in Belfast Trust service areas
  • Reviewing and reporting on the use of Encompass to identify carers in service areas and electronic referrals made to the Carer Support Service for the Belfast Trust Carer Register
  • Reporting annually on outcomes on the development and implementation of a Communications, Diversity and Inclusion Plan
  • Reporting annually on the range of stakeholders that Belfast Trust has engaged with including unpaid carers from ethnically diverse communities, young carers, faith communities, statutory services and the community and voluntary sector
  • Collecting data on the range of outreach events, information provision and implementation of Carer-i-Hubs (One Point of Contact for Information) in Belfast Trust service areas
  • Monitoring the implementation of ‘Carer Awareness’ education for staff across Belfast Trust service areas
  • Developing and implementing a Young Carers Action Plan and reporting on the work of the Young Carers Working Group
  • Implementing feedback to assess and improve quality of experience of carers
  • Promoting and making the offer of the Belfast Trust Carer Support Service to carers when referred to a service.
