Priority 2: Navigate Carer Support

Identify the Support That Carers Require at Different Stages

We will achieve this by working towards the following indicators:

Strengthening the Belfast Trust Carer Support Service

  • Reviewing the Carer Support Service structure and map out the vision for future services to effectively implement Carer Strategy, provide high quality support and increase Carers Register
  • Evaluating and improving the quality of experience for carers using the Carer Support Service utilising real time feedback tools
  • Working locally & regionally with Encompass to transition the Carers Register to a digital platform.

 

Supporting Carers to Navigate Information and Support at Different Stages of Caring 

  • Co-producing a Carer Navigation Framework to provide the right information at the right time in the right place at different stages of caring
  • Mapping the different stages of caring in partnership with carers to understand the information and support needs of care-givers
  • Building and maintaining a centralised digital Carer-i-Hub (One point of Contact for Information) for carers and staff to navigate support

 

Mapping & Sharing Carer Supports Available in the Community & Voluntary Sector across Belfast

  • Mapping & building a Belfast Trust digital database of community and voluntary sector services for carers and make available on the Carer-i-Hub for staff and carers
  • Facilitating a Carer Staff Community of Interests Forum for the statutory, community and voluntary sectors to map and share carer supports in Belfast.

 

Creating Champion Roles in the Belfast Trust to Improve the Experience of Carers

  • Developing Belfast Trust Carer Champion roles within each service area to improve the experience of carers with better information and support
  • Engaging with all staff and teams to become ‘Carer Aware’

 


We will measure our progress by:

  • Reviewing the structure and provision of the Carer Support Service and making recommendations for future service delivery to implement the Carer Strategy to meet the support needs of carers and increase the Carer Register
  • Monitoring and reporting on the 10% target of increasing the Carer Register annually
  • Providing bi-annual reporting from the Encompass regional group to monitor the transition of the Belfast Trust to a digital Carer Register
  • Reporting annual progress on the coproduction of a Carer Navigation Framework and implementation within service areas
  • Reporting annual outcomes of the numbers of Carer-i-Hubs (One Point of Contact for Information) available in service areas across the Belfast Trust
  • Reporting on the range of improvements in the range of information available for carers in service areas
  • Reporting annually on the number and outcomes of the Carer Staff Community of Interests Forum in partnership with the statutory, community and voluntary sector
  • Reporting outcomes on the progress of implementing the Carer Champion role within Belfast Trust service areas
