 Support Carers to Protect their Health & Wellbeing

We will achieve this by working towards the following indicators:

Developing a Carer Support Framework

  • Co-producing a Carer Support Framework and training to establish consistent Carer Assessment standards to improve the quality of experience for carers
  • Auditing the quality of Carer Assessments and implementing findings within service areas to improve the quality of experience for carers
  • Work locally and regionally with Encompass to transition to improved digital recording of Carer Assessments

 

Reviewing and Enhancing the Range of Responsive Respite Supports to Enable Carers to Continue Care 

  • Carrying out mapping exercises in partnership with carers to update information on available short break supports
  • Reviewing and embedding improved support for families at hospital discharge
  • Promoting Self Directed Support/Direct Payments options to increase usage in the Belfast Trust service areas.

 

Delivering a Programme of Events Supporting Carer Emotional, Physical and Social Health & Wellbeing

  • Delivering a quality hybrid programme of activities to support carer emotional, physical and social health and wellbeing through the Carer Support Service
  • Providing opportunities for carers to access health and wellbeing sessions including WRAP Programmes (Wellbeing Recovery Action Plan)
  • Providing information for carers to access Carer Support Groups via the Belfast Trust Carer Support Service and other providers

 

Identifying and Providing Support Mechanisms to Ensure Emotional Wellbeing of Carers is Supported

  • Providing a quality Belfast Trust Carer Counselling Service responsive to carer needs when in crisis and to support emotional wellbeing
  • Provision of a listening ear service via the Belfast Trust Carer Support Service
  • Map and share available crisis and advocacy support services to carers

We will measure our progress through the following metrics:

  • Reporting annual progress on the co-production of a Carer Support Framework to establish overall quality standards for carrying out Carer Assessments
  • Reporting on attendance of staff at Carer Assessment training
  • Reporting annually on the quality and quantity of Carer Assessments through regular audit processes and implementing service improvements based on findings
  • Reporting progress of transitioning to the new Encompass system to accurately record activity for Carer Assessments, Re-Assessments and declined assessments
  • Providing data and reporting on the numbers of carers accessing short breaks, Self-Directed Support/Direct Payments
  • Reporting annually on the performance of service areas to offer and complete Carer Assessments in the Belfast Trust
  • Reporting improvements from service areas on support available for carers at hospital discharge
  • Reporting feedback on the implementation of the Care Partnership pilot in acute settings and the offer of the Belfast Trust Carer Register to Care Partners
  • Providing data on the number of attendances at the Belfast Trust Hello Carers Activity Programme / short breaks to support emotional, physical and social health and wellbeing of carers
  • Reporting on the range of opportunities for carers to access health and wellbeing sessions including WRAP Programmes (Wellbeing Recovery Action Plan)
  • Providing quarterly data on the number of referrals to the Belfast Trust Carer Counselling Service, Listening Ear Service, Complementary Therapies and outcomes for carers
