What the service does
We provide a significant range of support services to people in their later years. If you are concerned that you or a loved one are not coping with some aspect of your daily life, we have social services for older people.

We have social workers based in a number of community and hospital settings. They can work with you to identify the help you may need.
Community Social Work Teams
When you are referred to the service, a social worker will become involved. Social workers are professionally qualified and their role is to support you in getting the care you need. Their aim is to promote independence and quality of life for all adults.
The social worker will work alongside you and your family to:
- find out which aspects of daily living you need support with
- agree on how this help can be provided
The social worker will complete a needs assessment. This may involve a discussion with you about a number of things, including:
- what you need to help you live well
- what or who is important to you
- any health, welfare, social and spiritual needs you may have
- your daily living activities, for example making meals, personal care
- what you are finding difficult and any concerns you may have
- support from friends and family
- carer’s support and assessment
- adult safeguarding, if you are concerned that someone is being mistreated or abused
The help and support you require may be provided by the Trust or other voluntary services, but always with your agreement and consent.
We always aim to support people in their own homes, but if this is not possible, other options may need to be considered such as supported housing or a care home placement.
How to access the service
You can refer yourself or be referred by a family member, GP or other health professional.
All referrals are handled by a call management centre, which is open:
- 30am to 5pm, Monday to Thursday
- 30am to 4.30pm on Friday
District nursing in South and East Belfast provides a 24-hour service.

Out of hours service
- The Regional Emergency Social Work Service (RESWS) is available if you need to talk to a social worker in the evening, at the weekend or on a public holiday.
- GP out of hours
- If you ever feel in immediate danger or think someone else may be in immediate danger, dial 999.
